PDF and Word (DOCX) downloads on OrderFormTemplates.com are provided free for business, school, and personal use. You may edit, print, and share completed forms with your customers or team. You may not resell our files as standalone template products or claim them as your original design work.

Third-party online forms

Links to forms.app, Jotform, and 123FormBuilder open on those providers' websites. Their terms, privacy policies, and pricing apply when you create an account or publish a form there. We do not control those services.
